Intertrochanteric fractures occur below the femoral neck. Unlike femoral fractures, intertrochanteric fractures are less likely to endanger the hip joint with a loss of blood flow. This type of fracture affects the broad portion of the femur. Two bone protrusions, called trochanters, provide a place for the leg muscles and buttock muscles to attach to the thigh bone.
The symptoms of an intertrochanteric hip fracture resemble the symptoms of other hip fractures, with some additional considerations. Pain in the hip area and weakness in the legs can signal a broken hip, especially in older patients following a fall. Sometimes, lightheadedness and decreased blood pressure can signal an intertrochanteric fracture. This can occur if the blood vessels are damaged during the injury, leading to bleeding in the hip. The presence of large bruises around the hip area is another sign of damage to the blood vessels.
Treatment for intertrochanteric hip fractures involves surgery. Hip replacement is seldom called for; the injury can be repaired by affixing plates and screws to the patient's original bone without the need for prosthetic parts. Before the surgery, doctors will place the patient in the best possible position to realign the bone. After an x-ray confirms the bone is in a good place, the surgeon will make an incision into the thigh and work to place a small metal plate on the outer part of the femur. The plate is secured into place with several small screws.
Recovery time usually lasts for about 12 weeks, although most patients have been walking for quiet a while by that point. Patients are encouraged to get back on their feet as soon as they can after surgery.
Preventing broken hip injuries is especially important. For older adults, the risk of hip fractures increases with the development of osteoporosis. Patients whose bones have been weakened by the presence of this condition can suffer a hip fracture from a relatively inconsequential fall.
