Plavix may increase risk of heart attack, stroke for some MO patients

Plavix is a popular cholesterol drug that keeps blood platelets from sticking together and forming a clot. Clots are what cause strokes and most heart attacks. However, in 2-14% of patients, Plavix can have the opposite effect and actually increase the risk of a heart attack or stroke.
 
Patients with low levels of the liver enzyme CYP2C19 are unable to properly metabolize the drug.  In these patients, Plavix cannot be effectively converted into its active form, so it does not work. According to Bristol-Meyers, about 2% of whites, 4% of blacks and 14% of Chinese patients are poor metabolizers of Plavix.
 
The FDA announced this week that Plavix will have to carry a black box warning about reduced effectiveness in patients who are poor metabolizers of Plavix. The FDA says these patients may be able to use aspirin instead.
 
Doctors can use a genetic test to determine if a patient will not metabolize correctly. The test costs about $500 and may be too costly for those without insurance.
 
This is the second label change for Plavix. Last year, researchers found that patients who used Plavix with the heartburn drugs Nexium, Prevacid, Protonix and Prilosec OTC were at an increased risk for heart attacks.
 
Plavix is the second best-selling drug in the world.
