Recent reports suggest that despite the efforts of medical institutions such as the Joint Commission of Chicago, medical mistakes including wrong site surgeries are increasing when they should be decreasing. In 2004, the Joint Commission, led by Dennis O'Leary, pushed for a 'universal medical protocol' aimed at using what was called "very simple stuff" to prevent what are deemed to very preventable accidents during medical procedures. But last year over 90 cases of wrong-site surgeries were reported-- up from 49 in '04.
